Complete the online forms to register a child to become a patient at St Augustine's Surgery.

You will need to fill in all of the details to register at our surgery. 

Please complete this form in full for anyone under the age of 16.

Please bring a copy of your child's immunisation history to the surgery on your first visit.

Register as child a new patient online

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery's catchment area, you will be asked to register with a surgery that's closer to your new home address and be given 30 days notice.

You can use our Online Form to let us know about a change in your address.

Please also let us know if your telephone number or email address changes.

A GP surgery can refuse to register you if:

• they're not accepting new patients
• you live outside their area and they only accept patients inside this area
• you have been removed from that surgery before

You can use your postcode or location to check that you’re within the surgery’s catchment area.
