NEW APPOINTMENT SYSTEM – WHAT YOU NEED TO KNOW

🚨 Paper triage request forms will no longer be accepted from 1st July 2025
only exceptional circumstances will apply*🚨
Paper Forms
We have seen an increase in patients requesting paper forms or taking them away. This poses challenges such as: prioritising your telephone calls, confidentiality in waiting areas, time taken by receptionist to transcribe your form and lost forms.
📢 For these reasons, paper forms will no longer be routinely issued to patients to complete from 1st July 2025.
What if I need extra support?
- Parents or carers can submit forms behalf of someone else.
- Call the practice - our receptionists are available to assist you. Someone else can call for you, provided if they have your permission.
- Easy read formats are available.
- In exceptional circumstances e.g., communication difficulties our reception team will help patients to complete the online form in person.
A Change in Process
The real change to our appointment system is in the process.
Our new system has already started to:
✔ Reduce phone queue times
✔ Enhance efficiency
✔ Improve accessibility for patients
✔ Provide flexibility in how we contact you (text, phone)
What is the new appointment system?
Our new process ensures all GP appointment requests and medical queries are reviewed before an appointment is booked. This helps us use our practice team and resources more effectively – which in turn provides greater access for patients.
Are there any exceptions?
- Nurse appointments can still be booked via reception by calling the practice.
How do I request an appointment?
- Submit your request via our online form available on our practice website.
- Use the NHS login or online equivalent: Log in - NHS App Online (service.nhs.uk)
- Call the practice as usual.
CAPACITY LIMITS:
We aim to keep requests open for as long as possible. However, if we receive more requests than we can process safety in a day, you may be signposted to alternative services until the following morning.
