Important information for Patients when submitting medical and admin requests.

We still have the same capacity / number of doctors & nurses each day so there will be times when we reach capacity and the medical form is closed. In these cases it will reopen the next day at 8 am. If you query is urgent you may need to call reception or contact 111 for help.

Results
If you are chasing a result requested by a doctor, please look at your online account / NHS app if you can. Results will be filed by GP's once they have been reviewed. https://staugustinessurgery.nhs.uk/services/get-test-results

Any results / letters from hospitals will be uploaded when we receive them. If you are worried please contact the hospital directly - they should communicate results to you directly.

Letter / Forms
If you have an admin query please give as much information as possible e.g. if you need a letter / form please say give as much detail as possible.

Referrals

Fit Notes
Again if requesting a new FIT note (sick note / Med 3) please give as much information as possible for the GP to save time.

Confidentiality
Please be aware that we do have a strict policy regarding confidentiality and data protection. Although can submit a request on behalf of some else we will communicate with that person directly and it will be saved in their notes.

This means that we can only give out information to the person the information relate to unless that person has given permission or are not capable of understanding them.
