Patient Notice - Home Blood Pressure Readings

We are still receiving blood pressure readings in many different formats – post-it notes, receipts, and scraps of paper. These are often difficult to read and record correctly.

To help us keep your records accurate, please use our official blood pressure form.

Please also note that we only require a total of 14 readings - 2 each day for 7 days (as stated on our official blood pressure form). Additional readings provided beyond this are disregarded.

You can request a copy at reception or this can be sent to you via mobile / email.
