Triage Forms

Please do not use the admin form titled 'I have an admin or routine care request' for medical matters, or to request a GP appointment. This form is for nurse appointment bookings or other administrative queries.

There will be times when the medical form is unavailable due to demand or outside of office hours. If closed, the medical form will be available at 8am the next day.

If you query is urgent please call the surgery. Submitting an admin form does not bypass the system and will be rejected.
