You can now contact the surgery using accurx via the link below or using the NHS app. 

From 30th September you can use the form to submit a medical request or admin query as part of our new appointment system. All requests for GP appointments will be processed this way so no one way will be quicker than another. 

Admin Requests

Accurx can help with admin queries; 

Selecting this option will allow you to:

  • Request a doctor's note

  • Request a fit (sick) note

  • Change your contact details

  • Request your Repeat Prescription

  • Follow-up on Test Results (Blood Test, Scans, etc.)

  • Any other advice or information that is not medically related

Medical Requests

From 30th September you will be able to use Accurx to;

  • book an appointment to discuss a new or ongoing medical problem
  • discuss some test results
  • get a skin lesion or rash checked
  • get some travel advice
  • get some contraception or HRT advice
  • request a home visit
  • book a ‘special appointment’
    • joint injection
    • contraceptive coil change
    • contraceptive implant change
Prescriptions

You can request repeat prescriptions, find local pharmacies. 

From the 30th September you can :

  • discuss my medication as I think it needs changing
  • ask for some medication that I haven’t had before
  • ask for some medication that I have had before but isn’t on my repeat prescription
Can a friend, relative, or Carer do the form for me?

Yes. On the form, there is a box where we ask you if the form is being completed by somebody else. Please note that we will only be able to reply to yourself regarding your care as the patient unless there is confirmed consent on your medical record.

Do I need to download anything or have a password?

No. If you do not want to download anything, you do not need to download any application or have a username and password to use this form.

The forms are also accessible via NHS app or via NHS website, if you have this set up.

What times of day is the form available?

The form will be available for completion from 8am to 5 pm Monday to Friday. This is to allow our clinical team to ensure we can review and triage appropriately each day. If you have a routine request after this time, you will need to wait until 8am the following day and complete the request.

If you are requiring urgent medical assistance or have a life-threatening emergency, you will still need to contact the emergency services or visit the hospital - Find urgent and emergency care services - NHS (www.nhs.uk)

What about my data?

Accurx is approved by NHS England to be used by the GP practices and the other systems involved in patient care. NHS England has a lengthy assurance process to make sure we meet the highest standards of safety and security. Your data is safe and is shared only with your GP Practice for the purposes of your direct care. Your data is stored and sent securely using industry best practices, and we only collect the data that is necessary to allow your GP Practice to provide you with care.
